Qualifications

Responsibilities

• Conducts psychosocial assessments for neonatal to geriatric patients to evaluate emotional, social, and financial needs.
• Provides counseling, support, and resource education to patients and families.
• Participates in discharge planning by identifying psychosocial barriers and coordinating post-acute care services.
• Facilitates referrals to internal and external services including mental health providers and social service agencies.
• Manages high-risk referrals and complex psychosocial cases, including new diagnoses and end-of-life care.
• Provides crisis intervention and support for acute distress, grief, or trauma.
• Collaborates with physicians, nurses, and case managers to support care planning and safe discharge.
• Participates in quality improvement and psychosocial outcomes tracking.
• Maintains timely documentation in the medical record per standards.
• Serves as a resource for peers and mentors other social workers.
