Qualifications

• Master's Degree in Social Work required; IL licensure (LSW/LCSW) preferred.
• 1–2 years of social work experience in an acute care setting preferred.
• Ability to communicate effectively with diverse stakeholders.
• Knowledge of community resources and care coordination best practices.

Responsibilities

• Collaborates with multi-disciplinary team to support patients with chronic illness and co-morbidities.
• Adheres to care management standards with evidence-based protocols.
• Coordinates care across departments, clinics, and community resources.
• Applies social work methods to assess, educate, collaborate, and coordinate resources.
• Provides support services to Endeavor patients and value-based contract members.
• Initiates advance care planning and goals-of-care discussions as appropriate.
• Ensures safe, budgets-conscious care with timely transitions and utilization management.
