Job summary

Qualifications

• MSW from an accredited program required.
• BSW may be considered with relevant experience and language proficiency.
• Minimum of 1 year experience with frail or elderly populations.
• California Social Worker license (MSW – Medical Social Worker) required.

Responsibilities

• Provide psychosocial assessments and develop care plans.
• Deliver individual counseling and coordinate with caregivers and teams.
• Identify needs, refer, and follow up on services for patients.
• Discharge planning and transition coordination.
• Engage with community resources and conduct caregiver workshops.
• Document promptly and participate in team meetings.
