Piedmont is hiring for a Social Work role focusing on patient and family wellness with a strong emphasis on care transitions in Inpatient and Emergency Department settings. The position requires a Master’s in Social Work from an accredited program and experience is not required, with ACM or CCM preferred.

You'll collaborate with physicians, care teams, patients and families to promote quality care and effective discharge planning across hospital services.

Qualificações

• Master's Degree in Social Work from an accredited social work program is required.
• No experience required.
• Accredited Case Manager (ACM) preferred or CCM - Certified Case Manager preferred.

Responsabilidades

• Promote patient and family wellness and improve care outcomes.
• Manage and develop comprehensive transitional care plans for patients with complex psychosocial needs in Inpatient and ED environments.
• Collaborate with service team members, physicians, patients, external agencies and families to ensure quality transitions and care planning.
