Job summary

Moberly Regional Medical Center is seeking a licensed Master Social Worker (MSW) to deliver clinical social work services, assessments, counseling, and care coordination for patients and families. The role supports discharge planning and links patients to community resources.

The MSW will collaborate with physicians, nurses, and case managers, handling diverse psychosocial needs, crisis interventions, and documentation in electronic medical records in a fast-paced hospital setting.

Qualifications

• Knowledge of social work principles, case management, and assessments.
• Strong counseling and communication skills.
• Ability to coordinate care and document in EMRs.

Responsibilities

• Perform psychosocial assessments across age groups.
• Provide counseling and emotional support to patients and families.
• Assist with discharge planning and post-acute care coordination.
• Facilitate referrals to social services and community resources.
• Manage high-risk cases and crisis intervention as needed.
• Document thoroughly in the medical record and participate in quality improvement.
