Responsibilities

• Conducts and documents detailed psychosocial assessments/evaluations to interpret social, environmental, and medical needs of the patient/family.
• Reassesses, monitors, and documents needs and interventions; ensures medical coverage documentation is obtained.
• Provides or supervises plan of care interventions including resource/referral, crisis intervention, education, patient advocacy, and counseling.
• Collaborates with other healthcare team members to facilitate interdisciplinary care and positive outcomes.
• Participates in performance improvement initiatives and related committee work.
