Job summary

Providence Willamette Falls Medical Center in Oregon City is seeking a Social Worker with an MSW to join the Social Work Services team. This full-time, day-shift role offers an opportunity to assess and intervene with patients and their support networks across the care continuum.

Qualifications

• Master's Degree in Social Work required.
• Oregon CSWA license within 60 days of hire.
• 2 years clinical healthcare, hospice and/or bereavement experience preferred.

Responsibilities

• Provide biopsychosocial/spiritual assessments and interventions.
• Collaborate with the IDT to develop and implement evidence-informed person-centered care plans across the care continuum.
• Advocate for patients to reduce health disparities and adhere to NASW ethics and SOP.
