Providence Willamette Falls Medical Center in Oregon City seeks a Social Worker MSW for Social Work Services. This full-time, day-shift role runs Tuesday-Saturday, 8AM-4:30PM, focusing on biopsychosocial assessments and evidence-informed care planning across the care continuum.

The CSWA will collaborate with the patient and support system to develop person-centered plans, in line with NASW standards and an IDT approach. Providence supports growth and diversity in a patient-focused culture.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Within 60 days of hire, Oregon Clinical Social Work Associate License eligibility.

Responsabilités

• Provide biopsychosocial/spiritual assessment and care planning.
• Collaborate with the IDT to implement person-centered care across the care continuum.
