Job summary

Providence Willamette Falls Medical Center in Oregon is seeking a Social Worker MSW to join the Social Work Services team. The role is full-time, day shift, Tuesday-Saturday, 8:00 AM-4:30 PM, with direct supervision and a focus on biopsychosocial assessment and care planning.

The CSWA will collaborate with an interdisciplinary team to develop evidence-informed, person-centered care plans, advocate for patients, and work to reduce health disparities while upholding NASW ethics.

Qualifications

• Must have a Master’s Degree in Social Work.
• Oregon CSW Associate License within 60 days of hire is required.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and intervention.
• Collaborate with the IDT to develop and implement person-centered care plans.
• Advocate for patients and families to reduce disparities and improve outcomes.
