Qualifications

• Master's Degree in Social Work.
• Within 60 days of hire Oregon Clinical Social Work Associate License.

Responsibilities

• Perform biopsychosocial/spiritual assessments and develop care plans with the IDT.
• Collaborate with patients and support systems to implement evidence-informed, person-centered care across the care continuum.
• Advocate to reduce health disparities and uphold NASW code of ethics and SOP.
