Job summary

The CSWA Social Worker, under direct and continuous supervision, provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy for patients and their support systems at Providence Willamette Falls Medical Center. We value patient-centered care and collaboration with an Inter/Multi-Disciplinary Team to develop evidence-informed care plans.

The role requires a MSW and Oregon CSW Associate license within 60 days.

Qualifications

• Master's Degree in Social Work.
• Within 60 days of hire Oregon Clinical Social Work Associate License.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy for patients and their support systems.
• Collaborate with the Inter/Multi-Disciplinary Team to develop person-centered care plans across the care continuum.
• Respect patient self-determination and adhere to NASW ethics and SOP.

Education
