Providence Milwaukie Hospital is seeking a Social Worker MSW for the Social Work Services team. The CSWA will perform biopsychosocial/spiritual assessments and evidence-informed interventions, collaborating with patients and families to advocate and coordinate care across the care continuum.

The role emphasizes reducing health disparities, adhering to NASW SOP, and working with the Inter/Multi-Disciplinary Team to implement plans that support patient self-determination.

Qualificações

• Master's Degree in Social Worker.
• Oregon Clinical Social Work Associate License must be obtained within 60 days of hire.

Responsabilidades

• Provide comprehensive biopsychosocial/spiritual assessment and treatment/interventions/advocacy.
• Collaborate with patient and support system to develop and implement evidence-informed, person-centered care plans with the IDT.
