Job summary

Advocate Health in Charlotte, NC is seeking a Part-time Social Work Specialist to perform psychosocial assessments and coordinate post-hospital care within the Clinical Care Management team.

The position covers overnight weekend shifts (Sat-Sun, 7:00 pm–7:30 am), with on-call duties and a pay range of $30.70–$46.05 per hour. A Master’s in Social Work is required and an LCSW is preferred.

Qualifications

• Masters in Social Work required; LCSW preferred.
• At least 1 year hospital or health setting experience preferred.
• Adherence to NASW Code of Ethics.
• Experience with Data Management Tools.

Responsibilities

• Completes psychosocial assessments for post-hospital care.
• Assists patients in coping with stress related to hospitalization.
• Assesses level of care or environment setting with care management team.
• Serves as a resource on emotional and social aspects of illness.
• Educates patients on community resources and post-hospital care options.
• Reports suspected abuse/neglect/exploitation and liaises with DSS.
• Responds to after-hours consult requests.
