Job summary

The role requires a Master’s in Social Work and preferably an active LCSW licensure, with hospital experience and knowledge of data management tools.

Qualifications

• Master's degree in Social Work required.
• LCSW preferred; licensure requirements may apply in SC.
• Minimum 1 year hospital/health setting experience preferred.
• Experience with data management tools.

Responsibilities

• Interviews patient and social support to complete psychosocial evaluation.
• Provides counseling and education to patient and family.
• Collaborates with multidisciplinary team to plan care.
• Serves as resource to staff on emotional and social aspects of illness.
• Reports abuse/neglect/exploitation as required.
• Maintains case records per departmental standards.
• Identifies barriers to transition across the healthcare continuum.
• Maintains knowledge of community resources and referrals.
