Job summary

Advocate Health in Charlotte, NC seeks a part-time Social Worker to provide education and support to patients, families, and staff. You will assess needs, plan interventions, and coordinate care across the continuum to empower patients and families.

Requires a Master of Social Work with potential LCSW; NC Certification preferred. Hospital experience is preferred, with responsibilities including assessments, referrals, and collaboration with the Clinical Care Management team.

Qualifications

• Masters in Social Work required.
• LCSW preferred.
• NC Certification preferred.
• Adherence to NASW Code of Ethics.
• At least 1 year of professional hospital or health setting experience.

Responsibilities

• Completes psychosocial assessments for post-hospital care.
• Assists patients in coping with stress related to hospitalization.
• Collaborates with Care Management to assess level of care needs.
• Serves as a resource to staff and physicians on psychosocial aspects.
• Provides information on community resources and makes referrals.
• Reports suspected abuse/neglect and liaises with DSS during evaluation.
• Responds to after-hours consult requests.
