Job summary

HCA Houston Healthcare Clear Lake is seeking a Social Worker (MSW) to perform non-clinical, biopsychosocial assessments and interventions on inpatient floors and in the emergency department. The role includes crisis intervention, discharge planning and referrals to financial assistance, with a focus on patient and family-centered care.

Qualifications

• Master's degree required.
• LCSW or LMSW/RCSW-I licensure preferred.
• No travel required.
• No experience required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance staff and others to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families and caregivers; support patient preferences and values.
• Provide psychosocial support, crisis intervention, education, counseling, advocacy and information referrals.
• Offer emotional support during end-of-life, terminal illness notifications and crisis interventions.
