Job summary

Overland Park Regional Medical Center, part of HCA Healthcare, is seeking a Social Worker PRN to join our inpatient and ED teams. The MSW performs biopsychosocial assessments, crisis intervention, discharge planning, and family support to help patients navigate complex care needs.

The role collaborates with nurses, physicians, and case managers; requires a Master’s Degree in Social Work, active state licensure, and either MSW field experience or internship.

Qualifications

• Master’s Degree in Social Work required.
• 1+ year of experience or MSW internship required.
• Licensed as a Registered Master of Social Work in the state(s) of practice, according to law and regulation required.
• Licensed Clinical Social Worker (LCWS) preferred.
• Certification in Social Work preferred.

Responsibilities

• Collaborate with care team to identify and address patient needs.
• Identify barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Coordinate meetings between the care team and patients/families/caregivers.
• Intervene for psychosocial needs, and support coping with chronic illness.
• Provide emotional support during complex/end-of-life situations.
