Job summary

St. David’s North Austin Medical Center, part of St. David’s HealthCare, seeks an MSW to perform non-clinical biopsychosocial assessments and interventions on inpatient floors and the ED.

The role includes crisis intervention, patient/family support, and referrals for financial or resource assistance to aid in discharge and post-acute placement. You will collaborate with RNs, physicians, and insurers to address barriers to discharge and recovery, while ensuring respectful alignment with patient

Qualifications

• Masters Degree required.
• LCSW, LMSW, RCSW-I, or LBSW license variants accepted.
• Minimum 3 years of experience required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers and staff to identify patient needs.
• Identify and address discharge barriers across financial, emotional, psychosocial, medical and spiritual domains.
• Maintain knowledge of Medicare/Medicaid and private payer regulations and length-of-stay concepts.
• Perform assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care teams and patients/families to respect preferences and values.
• Provide psychosocial support and referrals for emotional, social and financial needs.
• Offer emotional support during end-of-life scenarios and crisis interventions.
