The role involves collaboration with RN case managers, physicians, insurers, and the care team to address medical and psychosocial barriers to discharge while upholding patient preferences and values.

Qualifications

• MSW required and licensure or eligibility to obtain licensure.
• Experience or training in inpatient/outpatient social work settings.
• Knowledge of Medicare/Medicaid and managed care concepts is preferred.

Responsibilities

• Perform biopsychosocial assessments and develop discharge plans.
• Collaborate with care teams to address barriers to discharge.
• Provide emotional support and counseling to patients and families.
• Coordinate meetings with care teams and families.
• Assist with post-acute placement and resource referrals.
