St. David's Medical Center, part of St. David's HealthCare, is seeking a Social Worker (MSW) to perform biopsychosocial assessments and interventions on inpatient floors and in the ED.

The role emphasizes crisis intervention, discharge planning, and patient/family-centered care across units including ICU, Trauma, Oncology, NICU, Transplant and ED. Requirements include a Master's Degree in Social Work and an LCSW or LMSW/RCSW-I licensure.

Qualifikationen

• Masters Degree in Social Work is required.
• Licensed Social Worker status (LCSW) or LMSW/RCSW-I is required or pending licensure.

Aufgaben

• Perform biopsychosocial assessments and interventions for inpatients and ED.
• Provide crisis intervention and emotional support to patients and families.
• Identify barriers to discharge and coordinate appropriate discharge plans.
• Collaborate with RN case managers, physicians, insurers and care team.
• Assist with post-acute care placement and resource referrals as needed.
• Provide end-of-life support and counseling, as appropriate.
