Job summary

HCA Houston Healthcare Kingwood is seeking a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating post-acute services. You will liaise between patients and community resources, aiding in discharge planning across Med/Surg units.

The role requires collaboration with the care team to address barriers, perform assessments, and educate families about resources and coverage. No travel required.

Qualifications

• Evaluates psychosocial needs and coordinates community services for identified patients.
• Facilitates discharge planning and post-acute interventions aligned with care plans.
• Coordinates with care teams to address barriers to discharge and recovery.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers and staff to identify patient needs.
• Identify and address barriers to discharge including financial, emotional, psychosocial and medical factors.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Incorporate input from RN case managers to align discharge with patient care plans.
• Participate in interdisciplinary care planning and proactive discharge planning.
• Educate patients and caregivers on community resources and post-acute criteria and coverage.
• Coordinate meetings between care team and patients/families; support shared decision making.
