Job summary

InnovAge Holding Corp. seeks a Social Worker (MSW) to assess psychosocial needs, plan care, and serve as a liaison with the interdisciplinary team. The role involves on-site center work and field visits to participants’ homes to ensure continuity of care.

The position requires an MSW from a CSWE-accredited program and LMSW/LCSW licensure, with at least one year in a healthcare setting. Travel and driving are expected as part of duties.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
• Requires personal transportation and valid driver’s license.
• Minimum one year of healthcare or elder care experience.

Responsibilities

• Conduct psychosocial assessments and participate in care planning.
• Act as liaison between participant, family, and inter-disciplinary team.
• Document in Epic/EireneRx and complete required forms within timelines.
• Assist with discharge planning, guardianship where needed, and end-of-life coordination.
