Job summary

InnovAge is seeking a Social Worker, MSW to plan, organize, and implement social work services for participants and caregivers in accordance with InnovAge policies and regulations. This role conducts psychosocial assessments, participates in care planning, and liaises with the interdisciplinary team.

The position requires being on-site at the center and field visits to participants' homes, ensuring continuity of care and support.

Qualifications

• Master’s degree in social work from an CSWE-accredited program.
• Current state driver's license with reliable transportation.
• Minimum one year experience in healthcare.
• Experience conducting psychosocial assessments, care planning, and case management.

Responsibilities

• Conduct initial psychosocial assessments and participate in care planning.
• Act as liaison between participants and interdisciplinary team.
• Coordinate IDT meetings, care conferences, and discharge planning.
• Provide counseling and advocacy for participants and caregivers.
• Document in Epic, EireneRx, and other systems.
