Job summary

InnovAge seeks a Social Worker (MSW) to plan, organize, and implement social work services for participants and their caregivers in accordance with InnovAge policies and applicable regulations. This role combines on-site center work with field visits to participants' homes to ensure continuity of care.

You will perform psychosocial assessments, participate in care planning, document in EPIC/EireneRx, and act as a liaison between participants and the interdisciplinary team, supporting discharge

Qualifications

• Master’s degree from a CSWE-accredited program.
• One year experience in a health care setting.
• Experience conducting psychosocial assessments, care planning and case management.
• Requires personal transportation and a valid driver's license.
• CPR/BLS and First Aid required in California only.

Responsibilities

• Perform psychosocial assessments and participate in care planning and discharge planning.
• Serve as liaison between participant/family and the IDT to coordinate care.
• Document in EPIC and other systems and collaborate with RN Case Management.
• Participate in IDT meetings and in-home visits as required.
• Support end-of-life coordination and guardianship processes as needed.
