Job summary

InnovAge is seeking a Social Worker with an MSW to plan and implement psychosocial services for participants and caregivers. You will conduct assessments, participate in care planning, and act as a liaison with the interdisciplinary team and care facilities.

This on-site role includes field visits to participants' homes and coordination of transitions, with a focus on ensuring continuity of care and timely documentation in Epic and related systems.

Qualifications

• Master's degree in social work from an accredited program.
• Current LMSW or LCSW licensure (CA preferred).
• Minimum one year of health care or elder care experience.

Responsibilities

• Conduct initial psychosocial assessments and plan care with the interdisciplinary team.
• Coordinate housing, transportation, and transitions between facilities or home settings.
• Maintain accurate charting in Epic and related systems.
• Lead or participate in IDT meetings and care conferences.
• Provide counseling and advocacy to participants and caregivers.
