Job summary

InnovAge Holding Corp. is seeking a Social Worker, MSW to plan, organize and implement social work services for participants and caregivers, conducting psychosocial assessments, care planning and acting as liaison with the interdisciplinary team.

Qualifications

• Master’s degree from an accredited School of Social Work.
• LMSW or LCSW licensure required or pending as applicable.
• Minimum one year of experience in a healthcare setting; three years preferred.
• CPR/BLS and First Aid may be required in certain states.

Responsibilities

• Conduct psychosocial assessments and care planning with the interdisciplinary team.
• Perform in-home visits as needed to support continuity of care.
• Serve as liaison between participants, families and the IDT to resolve care issues.
• Coordinate discharge planning and transitions between residences or facilities.
