Job summary

One Senior Care is seeking a Master Social Worker (MSW) to support seniors and their families by guiding care plans, connecting to resources, and providing person-centered counseling. The role emphasizes empathy, communication, and collaboration within an interdisciplinary team.

The MSW will participate in assessments, care planning, and ongoing evaluations, lead group activities, and ensure accurate documentation while advocating for participants and coordinating resources across partners.

Qualifications

• MSW from an accredited school with one year of experience working with frail/elderly populations.
• Excellent communication and counseling skills with an empathetic, person-centered approach.
• Ability to coordinate with interdisciplinary teams and connect participants to resources.

Responsibilities

• Build trusting relationships with participants and families.
• Participate in initial assessments, care planning, and ongoing re-evaluations with the team.
• Provide individual and family counseling in a supportive, person-centered manner.
• Lead group discussions and activities to support emotional well-being and engagement.
• Document case management records accurately and timely.
• Coordinate with community resources and advocate for participants.
