• Assist patients and families with understanding care options and connecting to community resources.
• Complete psycho-social assessments for ESRD, Home Training, kidney transplant, and rehab patients.
• Provide crisis intervention and psycho-social support to patients and families.
• Educate staff and community on social work topics such as advance directives and resources.
