Job summary

The Medical University of South Carolina seeks a Social Worker (MSW) to support discharge planning and care transitions with the health care team. You will assess patients and caregivers during hospitalization and follow-up, identify post-discharge risks, and implement interventions to reduce those risks.

As part of the Revisions team, you will bridge discharge to outpatient providers and address questions from patients and families, ensuring smooth transitions and favorable outcomes.

Qualifications

• Master's degree in social work or social welfare program accredited by CSWE.
• MSW licensure required by SC Board of Social Work Examiners; certification in case management preferred.

Responsibilities

• Assist with discharge coordination to ensure a successful transition with the health care team.
• Complete patient and caregiver assessments during hospitalization and follow-up as needed.
• Identify discharge post-care risks and apply interventions to lower risk of post-discharge events.
• Bridge discharge to outpatient providers and address patient/caregiver questions.
