Job summary

Capital Health in Bordentown, NJ seeks a Social Worker (MSW) for the LIFE PACE program. This full-time role involves home visits, care planning with an interdisciplinary team, and counseling families to support aging participants.

Requires MSW, 2 years SW experience, elder care experience, and current social work license in-state; BLS certification and valid driver’s license expected. Eligible for comprehensive benefits.

Qualifications

• Master's degree in social work required.
• Two years of social work experience required.
• One year of experience with frail or elderly population.
• Current license in social work in the state.
• AHA BLS certification required.
• Valid driver's license required.

Responsibilities

• Conduct initial intake meetings with families and ongoing family meetings.
• Perform home visits to assess living environment and support systems.
• Develop and implement plan of care with the IDT.
• Provide individual and family counseling and group counseling as needed.
• Coordinate discharge planning and transitions of care.
• Advise on financial eligibility, MDPOA, and DNR directives.
• Liaise with external agencies (Aging, Social Security, Medicaid).
• Support participants and caregivers with crisis intervention and advocacy.
