NeighborHealth seeks a dedicated PACE Social Worker to join our Neighborhood PACE program, serving adults 55+. You will collaborate with an interdisciplinary team to assess needs, create care plans, and provide counseling and coordination at multiple sites in East Boston and Winthrop.

The role requires an MSW and pursuing LCSW licensure, with experience with older adults and community-based care. Travel between sites is expected as needed.

Qualificações

• MSW with at least 1 year of social work experience.
• Possess or working toward LCSW licensure.
• Experience with older adults and interdisciplinary teams preferred.

Responsabilidades

• Provide clinical social work and case management for Neighborhood PACE participants.
• Conduct psychosocial assessments; develop and update treatment plans with the IDT.
• Coordinate hospital, SNF admissions, discharges, and care transitions.
• Provide caregiver support and facilitate family meetings.
• Advocate for participants and maintain accurate documentation.
• Frequent local travel between sites as needed.
