This position supports Capital Health's mission by coordinating social work services within the PACE

Qualifications

• Master's degree in social work.
• Two years of social work experience, including care for frail or elderly populations.
• Current social work license in the relevant state; BLS and driver's license required.

Responsibilities

• Promotes collaborative relationships across divisions, building consensus and resolving conflicts.
• Manages department budget, procurement, and ensures compliance with applicable laws and ethics.
• Knows disaster recovery, crisis management, and business continuity; may work remotely from home.
• Participates in training, IDT care planning, and coordination for 24-hour care delivery.
• Conducts comprehensive assessments, develops care plans, and supports QAPI activities.
• Provides counseling to participants and families and coordinates discharge planning.
