Job summary

Capital Health is seeking a licensed MSW to join our care management team in New Jersey. The Social Worker will assess patient and family needs, develop discharge plans, and coordinate social and financial resources to support transitions from hospital to home or other settings.

The role requires strong communication, collaboration with multidisciplinary teams, and the ability to navigate complex barriers to care. Weekend commitments are part of the schedule.

Qualifications

• Master's degree in Social Work (MSW) is required.
• One year of clinical social work experience preferred.
• AHA BLS Healthcare Provider and Social Worker licensure required.
• Excellent communication, conflict management, organizational and planning skills.
• Clinical training in a medical environment; familiarity with case management systems.

Responsibilities

• Document clinical records and assess patient needs for discharge planning.
• Assess cultural, clinical, and social factors to plan care.
• Collaborate with health care team to identify barriers delaying transitions.
• Refer patients to social and financial resources post-discharge.
• Counsel on abuse/neglect, domestic violence and crisis interventions.
• Participate in staff development and performance improvement activities.
• Attend departmental meetings and hospital committees.
• Assist in data collection to support performance improvement.
