Responsabilidades

• Provide counseling and emotional support to patients and families.
• Coordinate post-acute care and discharge planning.
• Refer to internal and external support services and resources.
• Manage high-risk psychosocial cases and crisis intervention.
• Document accurately in medical records per standards.
