Qualifications

• MSW degree from CSWE-accredited program is required.
• 2-4 years of clinical social work experience in a healthcare setting preferred.

Responsibilities

• Conduct psychosocial assessments across age groups to identify needs.
• Provide counseling and emotional support to patients and families.
• Coordinate post-acute care and discharge planning.
• Refer to internal and external services for social support and financial assistance.
• Assist with crisis intervention and complex psychosocial cases.
• Document all activities accurately in the medical record.
