Qualifications

• No travel required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers and staff to address patient needs.
• Identify and address barriers to discharge: financial, emotional, psychosocial, medical, spiritual.
• Understand Medicare/Medicaid and private payer rules and length-of-stay concepts.
• Perform assessments and develop discharge plans for high-risk patients.
• Coordinate meetings with care teams, patients/families/caregivers and support patient preferences.
• Provide psychosocial support and referrals for emotional, financial needs.
• Offer emotional support during end-of-life, crisis and related situations.
