Job summary

Corewell Health's Dearborn Hospital is seeking a qualified Social Worker to join the Care Management team on a day shift. You will collaborate with an interdisciplinary group to assist patients and families with complex psychosocial needs, plan discharges, and coordinate resources across community agencies.

In this full-time role, you will conduct psychosocial assessments, document plans in the EMR, provide team support, stay current on regulations, and engage in committees as assigned.

Qualifications

• Master's Degree in Social Work required.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

• Conduct psychosocial assessments with patients and families to identify needs and plan care.
• Coordinate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the electronic medical record with accurate reflection of current condition and transition needs.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources.
• Participate in department, regional, or statewide committees as assigned.
• Stay current with federal and state regulations relevant to the role.
