Job summary

The Life Adjustment Team is seeking experienced Master’s level clinicians to join our team as Clinical Social Worker/Clinical Case Manager. The CCM coordinates care and provides case management plus community support for clients in home and community settings.

We offer supervision hours to qualifying candidates and support ongoing professional development. Successful candidates will implement our outpatient rehabilitation methods and communicate effectively with clients and families.

Qualifications

• Master's level clinician or licensed social worker (LMSW/LCSW/etc.).
• Excellent written and oral communication skills.
• Exceptional interpersonal abilities.
• Able to work a flexible schedule including evenings, weekends and holidays.
• Reliable transportation with valid insurance.
