Job summary

The Life Adjustment Team is seeking experienced Master’s level clinicians to join our team as Clinical Social Worker/Clinical Case Manager. The CCM coordinates care and provides case management plus community support for clients in home and community settings.

We offer supervision hours to qualifying candidates and support ongoing professional development. Successful candidates will implement our outpatient rehabilitation methods and communicate effectively with clients and families.

Qualifications

• Master's level clinician or licensed social worker (LMSW/LCSW/etc.).
• Excellent written and oral communication skills.
• Exceptional interpersonal abilities.
• Able to work a flexible schedule including evenings, weekends and holidays.
• Reliable transportation with valid insurance.

Our interdisciplinary team of rehabilitation specialists uses state-of-the-art training techniques to teach the necessary skills to recognize and alleviate symptoms and deficits associated with many forms of mental illness.

We're currently seeking experienced Masters Level clinicians who are passionate about making a difference to join our team as a Clinical Social Worker/ Clinical Case Manager. The CCM coordinates care and provides case management plus community support for clients both in their home and out in the community.

We're proud to offer SUPERVISION HOURS to qualifying candidates, so you can continue to develop your skills and grow as a professional. To be successful in this position, you must be able to learn and implement treatment programs using our unique outpatient rehabilitation methods. You should also have excellent written and oral communication skills, as well as exceptional interpersonal skills. Additionally, you must be available to work a flexible schedule (including occasional evening, weekend and holiday assignments) and have reliable transportation with insurance.
