TEEMA Group is seeking a dedicated Social Worker to provide compassionate psychosocial assessments, care coordination, and counseling for a panel of patients. You will collaborate with families, caregivers, and healthcare teams to implement individualized care plans and ensure access to community resources.

The role emphasizes teamwork, ethical practice, and timely documentation while maintaining patient-centered support and advocacy in a dynamic healthcare setting.

Qualifications

• Master’s degree in Social Work from an accredited institution required.
• Bachelor’s degree in Social Work may be considered with relevant experience and language proficiency.
• Minimum of one (1) year of experience working with frail or elderly populations.

Responsabilités

• Provide psychosocial assessments and develop individualized care plans for a panel of patients.
• Deliver case management, care coordination, and referrals to community resources.
• Offer individual counseling and facilitate family conferences and caregiver support groups.
• Advocate, coordinate, and follow up on patient support with external agencies and care facilities.
• Maintain thorough documentation and ensure compliance with ethical and legal standards.
