The University of Virginia Health System is seeking a Case Manager MSW to lead discharge planning within UVA Community Health. You will collaborate with physicians and the healthcare team to assess, plan, implement and evaluate proactive, safe discharge plans for complex patients.

As part of a multidisciplinary team, you will coordinate care, ensure post-acute options are appropriate, and maintain knowledge of community resources. Weekend and holiday coverage is required.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• 2+ years of professional social work or healthcare experience; case management preferred.
• Weekend and holiday coverage required.
• Proficiency in Microsoft Office products.

Responsibilities

• Lead the complex discharge planning process as part of the healthcare team.
• Collaborate with physicians and team members to assess, plan, implement and evaluate discharge plans.
• Coordinate care for psychosocially complex cases and serve as a resource for discharge planning.
