Job summary

The MSW/LCSW credential is required, with 2+ years of social work or healthcare experience preferred. Weekend and holiday coverage and strong communication skills are essential.

Qualifications

• Master's Degree in Social Work (MSW) from a CSWE-accredited program.
• 2+ years of professional social work experience or 2+ years in healthcare; case management experience preferred.
• Licensed Clinical Social Worker (LCSW) certification required.
• Weekend and holiday coverage required.
• Knowledge of Microsoft Office products and keyboarding preferred.

Responsibilities

• Lead complex discharge planning as part of the healthcare team.
• Collaborate with physicians and other providers to assess, plan, implement and evaluate discharge plans.
• Provide guidance and training to other case managers on new resources and programs.
