Job summary

Qualifications

• Minimum 1 year of clinical social work experience in a health care setting.
• Credentials from multiple states; primary state credentials required at hire; secondary state credentials post hire.
• Licensed Independent Social Worker (DC) within 6 months of hire OR Licensed Graduate Social Worker (DC) within 6 months of hire.
• Licensed Master Social Worker (Maryland) within 6 months of hire.
• Licensed Master’s Social Worker (Virginia) within 6 months of hire.

Responsibilities

• Collaborate with health care team to coordinate cost-effective services
• Complete psychosocial assessments evaluating goals, support, resources, health status, and treatment response
• Develop and monitor plans of care to promote optimal functioning and quality of life
• Identify, facilitate, and advocate for community resources meeting the plan of care
• Document case management activities in accordance with standards
• Analyze outcomes to identify program, quality, and cost improvements
• Assist with applications for local, state, and federally funded programs and refer when needed
• Provide supportive counseling and education to members, families, and caregivers
• Promote knowledge of current case management standards and laws governing social work in the locale
• Schedule family meetings and assist with guardianship paperwork when needed
