UVA Community Health is seeking a Case Manager BSW to support discharge planning and care coordination within its care teams. This role partners with physicians and other clinicians to design proactive, safe discharge plans for diverse patient populations.

The position emphasizes strong communication, clinical teaching, and organizational skills, with weekend/holiday coverage and rotation as part of a non-exempt schedule.

Qualifications

• Bachelor's Degree in Social Work required (CSWE accredited).
• 2+ years of professional social work or healthcare experience preferred; Case Management experience preferred.
• Virginia RN license or Social Work degree required; Case Management certification preferred.

Responsibilities

• Facilitate complex discharge planning as part of the healthcare team.
• Collaborate with physicians and healthcare staff to assess, plan, implement and evaluate discharge plans.
• Serve as discharge planning resource and coordinate post-acute care options.
• Maintain knowledge of community resources and ensure patient care continuity.
