Qualifications

• Graduate degree in social work (MSW) from CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) certification.
• 2+ years of professional social work or healthcare experience.
• Weekend and holiday coverage.
• Proficient in MS Office and basic keyboarding.

Responsibilities

• Lead complex discharge planning processes and coordinate care with the healthcare team.
• Collaborate with physicians and other team members to assess, plan, implement and evaluate discharge plans.
• Provide guidance and training on resources and programs; support care coordination for psychosocially complex cases.
