Job summary

Qualifications

• Graduate Degree, required. Master's Degree in Social Work.
• Two or more years professional social work experience or two or more years healthcare experience, preferred.
• Appropriate state approved curricula for restrictive interventions within 3 months of hire, required.
• Weekend and holiday coverage is required.

Responsibilities

• Lead patient assessment and complex discharge planning as a Case Manager MSW.
• Collaborate with physicians and healthcare team to coordinate and evaluate care plans.
• Develop proactive discharge plans and coordinate post-acute care options.
• Serve as discharge planning and care coordination resource for psychosocially complex cases.
