Job summary

West Virginia University Health System is seeking a case manager to join an interdisciplinary treatment team. Primary responsibilities include initial assessments, discharge planning, and coordinating post-acute services with minimal supervision, appropriate to patient age.

You will collect data, document in medical records, advocate for patients, coordinate referrals, and ensure smooth transitions with healthcare providers and community resources.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Nursing, or related field.
• State criminal background check and Federal (if applicable) background checks as required for regulated areas.
• One year healthcare experience preferred.

Responsibilities

• Participates as an integral professional member of an interdisciplinary team.
• Collects and records data comprehensively and accurately in assessments and progress notes.
• Assesses discharge planning needs and documents in the medical record.
• Contacts referral sources and health professionals involved in discharge status.
• Advocates for patients to maximize available services.
• Coordinates planning and assists in determining appropriateness of admissions and referrals for discharge planning.
• Maintains collaboration with patients and other healthcare providers.
