Job summary

Energy Jobline ZR is seeking a Care Manager to support Franciscan Friars across San Francisco, Oakland, Danville, San Juan Bautista and the greater Bay Area. This is a direct hire, full-time role that can be part-time in some cases, with one in-office day per week in San Francisco and some remote work.

The Care Manager will provide individualized and group support, coordinate care, and deliver monthly impact reports while organizing regional meetings. Clinical supervision is available to staff.

Qualifications

• MSW or related field required.
• 2 years of experience working with older adults or adults with complex medical and psychosocial needs.
• Valid driver's license, reliable vehicle, and willingness to travel within Bay Area counties. Mileage reimbursement provided.

Responsibilities

• Provide solution-oriented care management for Franciscan Friars.
• Proactive care coordination in a dynamic environment.
• Monthly reporting to stakeholders on impact measures.
• Convene regional meetings of participants; clinical supervision offered.
