Energy Jobline ZR is seeking a Care Manager to support Franciscan Friars across San Francisco, Oakland, Danville, San Juan Bautista and the greater Bay Area. This is a direct hire, full-time role that can be part-time in some cases, with one in-office day per week in San Francisco and some remote work.

The Care Manager will provide individualized and group support, coordinate care, and deliver monthly impact reports while organizing regional meetings. Clinical supervision is available to staff.

Competenze

• MSW or related field required.
• 2 years of experience working with older adults or adults with complex medical and psychosocial needs.
• Valid driver's license, reliable vehicle, and willingness to travel within Bay Area counties. Mileage reimbursement provided.

This is direct hire and full time, can be part time if . Caseload is 25 Franciscan Friars located in SF, Oakland, Danville, San Juan Bautista and the greater Bay Area. This requires 1 day/ week in SF office, community based work and some remote work.

The Care Manager serves Franciscan Friars who require a combination of individualized attention and group support. This role requires interpersonal intelligence, and the ability to provide solution-oriented care management in a dynamic and complex environment. Proactive care coordination is central to the daily duties of this role. Monthly reporting to stakeholders on impact measures along with convening regional meetings of participants is expected. Clinical supervision is offered.
