AdventHealth in Orlando, FL is seeking a Social Work Care Manager to support discharge planning and patient advocacy. You will coordinate care plans, educate families, and work with a multidisciplinary team to ensure timely transitions from hospital to post-acute care.

The role requires an MSW with at least four years in social work and experience in care management, discharge planning, and community resources. Located at AdventHealth Orlando, full-time night shift offers comprehensive benefits.

Competenze

• Master's in Social Work required and licensure guidance may apply.
• Minimum 4+ years of experience in social work and care management.
• Strong communication and discharge planning capabilities.

Mansioni

• Educate patients and families about illness impacts and available resources.
• Assess readmissions and reasons from patient/family perspective.
• Organize and facilitate care conferences with the multidisciplinary team.
• Document discharge plans, evaluations, barriers, and progress.
• Advocate for patient rights and support decisions during hospitalization.
• Coordinate post-acute care authorizations with payors as needed.
